Nephrogenic Systemic FibrosisForum -

(NSF) Nephrogenic systemic fibrosis is a rare and highly debilitating disorder that involves extensive thickening and hardening of the skin with fibrotic nodules and plaques.
MRI contrast media have very low side effects, but accumulating data indicate that gadolinium-based contrast agents increase the risk for the development of NSF among patients with severe renal insufficiency or renal dysfunction due to the hepato-renal syndrome or in the perioperative liver transplantation period.
Due to this reason, gadolinium contrast agents are now considered contraindicated in patients with an estimated glomerular filtration rate fewer than 30 mL/min/1.73m2. In these patients, avoid use of gadolinium-based contrast agents unless the diagnostic information is essential and not available with non-contrast enhanced magnetic resonance imaging (MRI).

Recognized or possibly associated factors for NSF:
Severe renal failure;
high dose of gadolinium chelate;
high dose of erythropoietin;
high serum phosphate levels;
high serum calcium levels;
major surgery, infection, vascular event;
history of hypothyroidism;
metabolic acidosis.

When administering a gadolinium-based contrast agent, do not exceed the recommended dose and allow a sufficient period of time for elimination of the contrast medium from the body prior to any readminstration. Screen all patients for renal dysfunction by obtaining a history and/or laboratory tests.

Nitroxide radicals (or nitroxyl spin labels) are stable organic compounds with theoretical potential for use as a paramagnetic MRI contrast agent. Similar to gadolinium they have an unpaired electron, a property that provides enhancement in T1 based MRI, and a comparable pharmacokinetic. Depending on their structure and chemical bonding, different nitroxides formula may have the potential for use as cardiovascular imaging agents, to enhance the MR imaging on joints (e.g., dendrimer-linked nitroxides have a strong affinity for cartilage), to evaluate brain tumors and infarction, and as a contrast enhancement agent of body/abdominal NMR imaging. Nitroxides are rapidly enzymatically reduced in tissues to products that do not enhance the NMR signal, which can be a problem for MR imaging. In animal experiments with EPRI (electron paramagnetic resonance imaging), tissue redox studies show differences between tumors and normal tissues, which reflect their respective redox status consistent with the reduction/clearance of nitroxides.

(NMR) Nuclear Magnetic Resonance is a physical phenomenon of the magnetic property of nuclei, which have a positive nuclear spin quantum number.
Under the influence of an external static magnetic field this nuclei will precess about the direction of the magnetic field with an angular frequency (Larmor frequency). Through absorption and emission of RF energy (gradients, RF coils) at the resonance frequency (Larmor equation) and the processing of this raw data by the Fourier transformation - physical, chemical, electronic, and structural information about molecules can be obtained (NMR Magnetic Resonance Spectroscopy, Magnetic Resonance Imaging).

(PARACEST) The alteration of the proton density or total water signal changes contrast and can be detected by the MRI scanner. Paramagnetic chemical exchange saturation transfer contrast agents are based upon the magnetization transfer mechanism.
Lanthanide ion complexes formed with tetra-amide based ligands display unusually slow water exchange kinetics and this feature may be used to alter image contrast by applying a selective presaturation pulse in an imaging sequence. This results in chemical exchange saturation transfer (CEST) from the lanthanide-bound water to bulk water thereby altering image contrast.
Chemical Exchange Saturation Transfer (CEST) agents are a class of contrast agents that could potentially revolutionize the MRI field because of their improved sensitivity and can have a great impact on functional magnetic resonance imaging.
